Prior to our first meeting, please print and fill-out the following forms and
bring them to your first sesssion.
You can fill them out on-line and print them, or print them off and fill them
in by hand.
| Note: If you fill out one of these forms in a web browser and then save it, your entries will not be saved. Only the form, not the entries, is saved in html. So fill out the form in web browser and print it, or, to keep a soft copy of the form for your records or for emailing the form, save the html file above using 'save-as' after opening the form in a web browser. Then open the file in a word processor that can read html (like Microsoft Word). Fill out the form and save it as a word processing document, not html. This document will have all entries and can be emailed or re-printed anytime. Remember that e-mail is not secure or confidential, so hand carrying the printed forms is safest. |
